Any Alternative ways to display information and interact with users

Am trying to create similar to this in CRM Now am creating dialogs but There are many fields which i should hide and show based on few conditions, like EIN(extra field) will be added and few fields will be hide based on few conditions so is their any other ways to achieve this except creation of dialogs??

    i have almost 400+ fields so is this the good way to go for those many fields and wont it be required almost equal effort of creating many dialogs since their i has to concentrate on validation and UI part as well which i can neglect in CRM dialogs..??

    Can you satisfy your requirement by creating more than one form and changing which form is displayed depending on your EIN or similar?

    these are two sample forms which i has to create i have more than 500+ different combination of  fields which i has to create in CRM and i have really short time is their any other ways to do so except creating a dialogs for each and every combination separately..??
