Dear MattU,
You can replace the out-of-the-box (OOB) icon by editing the form's HTML and applying custom CSS. There are several ways to do this, but the simplest method is by targeting the .onFormSubmittedFeedbackIcon class in your CSS.
.onFormSubmittedFeedbackIcon
First, either use a publicly accessible image URL or upload your custom icon to the Library Assets in Dynamics 365. Once uploaded, copy the public URL of the image.
Then, apply the following CSS to replace the default icon and also add copied URL in the css code:
