I'm facing an issue when creating an OTB task record in CRM (v 9.0) on premises. The task is creating as "Cancelled" or "Completed" sometimes. It seems this is an intermittent issue. Anyone faced this issue? Any workaround for this?

Can you check if you have any automated workflow/plugin configured on Task entity which is marking Task as completed/cancellled?
Are you creating task from Activity or from within any records like lead or contact?
Check if owner of the task is different than owner of the regarding record.

There is no any pre operation steps registered in task.
Task is creating through a custom plug in.
I haven't check the owner of regarding object. But anyway how does it impact creating the task as Completed or Cancelled?

If possible would you please share code you have to create task from plugin?
Are you also mapping Statecode and Statuscode while creating Task from plugin code.
Also, can you please share Audit History screen by going to Task record ->Related.
