Store Survey Responses on note entity

Hi Team,

I have trying to store the form pro response in the notes entity. facing below error. Kindly suggest me you valuable information.

      I have found the solution for this issue storing the forms pro response rating on case entity without using the custom entity

    Please note, The Flow which I have configured the successfully working. the ratting of the tickets not storing the Notes recored

    Create a note for response CDS options was missing. Instead of can I use create new record options.

    Hello,

    Sorry but I am not able to see the error, do you mean that it says "Overdue"?

    Also please review if the integration with Customer Voice is defined as mentioned in this link, maybe it could help just  to confirm if the regarding GUIDs are set in the proper spaces:
