Workflow Assistance

Our workflow has been functioning without any issues and it randomly began to malfunction, we have been receiving this prompt after every file submitted: " (file name) can no longer be found in the workspace, something went wrong. This workflow is now terminated and the approval process has come to a halt." 


My question is, where can I start to diagnose and fix this issue?
