How to Add Pickup Address & Delivery Address in Work Order of Field Service?

This is for a messenger service. The work order has a pickup address and delivery address.

In the work order there is only one address- Is this the delivery address?

If so where should the pickup address be given? Is this done by editing the form and adding a pickup address? if that is the case how can the two addresses be use to schedule the automated booking and also to calculate the pricing based on the distance between the pickup address and delivery address.
