Strange behavior of lookup fields inside Dialogs

I am using Dialogs. The version of CRM is following: 1612 (9.0.19.8) (DB 9.0.19.8) on-premises.

I use either Customer lookup or product lookup field.

From time to time something strange happen inside that dialog prompt step.
Lookup button becomes disabled. I still can't find what could be the reason. It doesn't happen all the time, but when this happen you cannot do anything, even if you go back one step, this button end input mask becomes unresponsive.

Is this some bug that is already registered? I don't now what to do in that cases because you cannot refresh dialog page, so you need to start new dialog session again from the start when this happens.

This fields are working usually but from time to time this strange behavior occurs.
