Canada Payroll - Box 45 will not print on T4s

Box 45 will not print on our T4s ( the new Dental Benefit),   It shows on the employees' T4s  but just won't print.on the actual T4.
Anyone else have this problem and was able to resolve ?  Thanks!

    I tested it and it works fine for me.
     
    I would make sure you do not have security set to a modified form, that would be a problem, you need to print the default form.

    Thanks Terry !   That is it  - we are using a modified form.  Not sure who had originally set this up , but I was able to just add P_Emp_Offer_Dental and it appears but it prints the actual wording "Payee" instead of the associated number.
    To reset to back to default, I  just need to delete these 2 modified forms ?   I don't have much experience in the modified reports area except changing field sizes.

    Instead of deleting reports, try to unmark them from alternated-modified report security window.
    If you will delete them , error messages pops up.

    Thanks Almas,  I did set the reports for the T4 to use the default, but the still had the same result.
    I appreciate your assistance. I will need to log a help ticket as it look likes we more issues than expected.
