Thanks Terry ! That is it - we are using a modified form. Not sure who had originally set this up , but I was able to just add P_Emp_Offer_Dental and it appears but it prints the actual wording "Payee" instead of the associated number.
To reset to back to default, I just need to delete these 2 modified forms ? I don't have much experience in the modified reports area except changing field sizes.
I would make sure you do not have security set to a modified form, that would be a problem, you need to print the default form.
