I have analyzed and tested several times for your question. Lookup fields can be added to the Form, you can customize them the way I did it. The method I got after testing is as follows:
1. First, you need to operate on these two Forms.
2. I'll use the /Owner/ and /Created By/ lookup fields as examples. You need to add the /Owner/ and /Created By/ lookup fields to the /Send Settings/ of both forms. How to add the fields: You need to click /Reply to email/ in one of the /Send Settings/, and then switch to the /Table columns/ from the /Tree view/ in the left navigation bar to select the two lookup fields /Owner/ and /Created By/.
After several attempts, I realized that the order of the added fields must be the same in both forms.
3. After doing what I did above in /Email header/ and /Information/ and publishing it, it successfully showed up in the app.
