if you know how to manipulate CDS, you can use the attract and applications entities and generate the survey invitation to the participant.
you can set the Forms Pro invite to have an anonymous answers.
that's all I can think of.

Yes that is an option.
But in that case, as I mentioned the Microsoft form is available to candidate once they submit the application. We cannot make it available on the same portal page where other candidate details are captured.
Candidate need to click on "View Application" and then our Microsoft form is available to capture additional info.
Also, the response is submitted anonymously so will have to ask the candidate to enter the name again in the form created.

You asked "I cannot embed or place it in the career web page" and I assumed you knew how to embed an iframe in your portal.
you should copy the generate code and paste it in your portal html code.
maybe I don't understand your question....

ANSWER: Yes, you can embed it or place it on a career page. When you try to send the survey from Forms Pro, you will get the Embedding code.
2. I submitted the response of the questionnaire form, but is unable to see the response.
ANSWER: if you installed the solution in CDS (PowerApps) you will find them in the "msfp_questionresponse" entity - Customer Voice survey question response, or centralized on the "Customer Voice survey response" entity
3 Embed in Career portal
ANSWER: Please see the first response. Forms Pro can be embedded on the the portal - Powerp Apps cannot be embedded since they require an username and a password in Azure AD

ANSWER: each question in Forms Pro will represent an actual field in CDS. If you get the answers in CDS, then with Workflows/Flow you will be able to update other entities.

You can create a Forms Pro form and include it in the Attract interview process. Then, these fields can update the profile in CDM and later in Core HR.
