Notes Card missing

Hi,
 
I've created a new Sandbox with Customer Service installed.
 
I want to modify the Activities display for Notes but OOTB, there is no Note Card form.
 
The option to create New is disabled on the Note, in Forms. 
 
How would I create a new Note Card?
