Hi,
I have created the simple model form in Enterprise Portal, created the new privilege, added the menu in the entry point and added the required tables in the permissions.
The user can see the menu now but once they click the menu the empty form is opening. If i give system administrator role then they can view the form. I am not sure that what i am missing.
Any idea?

Did you follow the document:
docs.microsoft.com/.../walkthrough-using-roles-and-privileges-to-control-access-to-forms
Set the properties of entry point:
Make the ObjectType of the entry point be MenuItemDisplay.
Make the ObjectName of the entry point be yourMenuItem.
Set the AccessLevel of the entry point to Read.
It looks fine. I set everything. In my case the object type is web url item and even i set delete permission to AccessLevel.
Did you pull the managed web content under your privilege as well? You can check the privilege "ActivitiesMainBasicView" as an example which has a few managed web controls along with the web url items.
