Hi,
we are configuring workflow email for Purchase Requisition line item approval.
Here's email template setting. We have only one email template.
The sent email got wrong subject & with additional content.
How do I troubleshoot this? How to check where the wrong subject & additional content came from?

Have you tried to debug set breakpoints in the SysMailerNet class? The Subject, body should be set here.
Go to System Administration > Users > {Your user id} > Options > Notifications > Workflow notifications.
After changing Line-Item Notification type to Individual, the email appeared as expected.
Naturally need to run the workflow again.
