Form not showing in Flow

I apologize if this has been answered before but I can't seem to find any solution to this. I may have this in wrong area as well.

I am trying to create a flow for dynamics 365 - I am just starting. I have created a form (test form) and it does not show up in the survey drop down and I have no idea how to reference it.

Any help will be appreciated instead of spinning my wheels any longer.

Thanks - I figure it out. I was using the old Froms instead of Froms Pro. Needed to use form Pro.

The trigger you mentioned is for the following product:

https://forms.office.com/

If here you mean to work with the Microsoft SharePoint Data Forms, then this is not supported.

Data Views inserted as forms can be created for SharePoint lists and libraries, database connections, and local XML documents. They do not, however, support SOAP and REST Web service connections.

While Flow is built under REST Web service connections.
