How did a record from INVENTTABLEMODULE get deleted?

Today one item number was not showing up in the Item Details form.

The problem was it was missing its InventTableModule record.

How did a record from INVENTTABLEMODULE get deleted?

It was Module type 1.  Purchase Order.

Is there a form where these can be deleted?
