Dynamics 365 Operations comes with 1 Tier-1 Sandbox. If the customer has 3 developers, does it mean that they only need to purchase 2 extra sandboxes ?
Customer needs 1 Tier-1 Sandbox to sync up the development with the Production Sandbox. So if the customer has 3 developers, do they need to purchase 3 extra sandboxes instead of 2?

Yes, you need to have 1 box per developer. You can buy 2 additional Tier-1 as an add-on, deploy them locally or deploy them to Azure. If devs have msdn subscription it could be quite cheap, please read wiki article for details ax.help.dynamics.com/.../development-and-continuous-delivery-faq
Also you may need to deploy build server as well, information about it could be found on wiki.
