I am trying to open my lead entity form in crm but it give the below error.This error is occoured after i have done the changes in field called status reason,I have deleted values & update some values in the same field,that's why lead form unable to open.Please can anyone help me on this issue.

The easiest solution will be to delete those leads.
The next one from the easiness standpoint - recover deleted status reasons, update leads that have those deleted/recovered status reasons with the status reason that you won't delete, delete status reasons again.
The next one from the easiness standpoint - export leads using Excel with the checkbox "Available for the update", include "Status Reason" field, locate those "affected" leads, update the "Status Reason" in the file to the "Status Reason" that is available, reimport the data back to the system.
