It's actually a simple GL entry which I tried to reproduce and couldn't make it happen again.
I validated the parameters you mention. All looks good.
I also validated if there was any other error posting to this account since the go live (which was in April) and there was only 1 entry which is the amount in question. Looks like a one time thing.
I guess there was a undefined glitch in the system at the moment of posting, that it could find some value so the behavior was to post to the automatic account.
I really don't like one time behaviors which aren't reproducible and I can't explain.
We will monitor closely.
