You can use this and the documentation as guide: https://docs.microsoft.com/en-us/dynamicsax-2012/appuser-itpro/development-tasks-for-reporting
You can also search for blog posts with some examples.
I think you should first check the working of the example you showed and work based on that.
Moreover,
They have used the menuitem to call the report to be run,
Please check the menuitem properties.

@AXTechie I have created a form and I want the all the value/data in the form to show on SSRS Report. How to make it? What I've seen in the AX 2012 they have done were using controller and contract class. But I've no idea how to do it.

They have written some code to connect to the report,please go through code.
Right click on the form and personlize it and you can check that code.
