Whenever I'm running a model-driven app, and reload the page, the current form (selected and ONLY available form for that entity in the app) is replaced by the default form. Has anyone else experienced a similar issue?

Yes, my test result is the same as yours.
Not sure if this is by design, I will raise a support ticket to report this issue.
If there is any progress, I will follow up here.
I've opened a support ticket as well. I'm fairly certain this isn't expected behavior as showing a different form could have data implications by exposing unwanted fields, changing business rules, etc.
Currently, we can achieve your needs by selecting two forms in app designer and setting the order of forms.
If you select two forms instead of one in the app designer, the two forms will be displayed normally in the app.
The order of these two forms is set in the customization.
So, before the bug is resolved, you can achieve your needs in this way.
