Customer Voice question in the SURVEY after submitting shows 'no response' in the REPORTS

Hello everyone,

I was trying to fill in question no. 4 in my customer voice form (under SURVEY_ in preview and submit. However in response (under REPORTS), for most of the users the question no. 4 has no response I downloaded all excel reports from customer voice, most of the responses for question 4 did not have a reponse. I have attached few screenshots. Please have a look. Any help would be appreciated.

This is the customer voice form (survey) - see question 4 - customer fills it but in the customer voice (reports) most of the the response for this question 4 for most of the user does not get populated exported all the responses in an excel sheet - circled in red column, has only 2 responses from question 4 - rest are blank
