So when you qualify a Lead, it creates a Contact with some basic information i.e. phone, email, etc. However, as far as I can tell this newly created Contact is not associated with the resulting Opportunity that is created from the Qualified Lead and therefore any contact information is lost in translation. This doesn't seem right.

As per the documentation, it should not happen. Can you please check the audit history may be a user disassociated contact from the opportunity.

No, it has not been disassociated. What is also strange is that it is not updating the 'Originating Lead' field in the Contact either whereas it is doing this in the Opportunity record.
