How do I delete (not hide) one of these forms?
Hi mattheba,
You could not delete form from App Design.
Please navigate to Data->Tables->Case,
Click Case and select the form you want to delete:
Or expand entities then select Case entity to delete it:

You cannot delete form from the app. You have to go to solution -> Entity -> Forms and Delete the form you want. Before that you have to remove all dependencies. So you have to unselect in model driven app first.

Hi,
It is always a good idea/recommendation to deactivate them instead of deleting them from the system.
So you deactivate those forms from maker portal by navigating to Tables->Select your table->Forms->Deactivate
Now you will unselect those forms from your app designer.
