Is Customer Voice available for 365 On-Premise Deployment?
Customer voice is available as online service only.

We are using dynamics 365 v8.2 on-premise. Can we integrate "Customer Voice" to our deployment?
Actually,We would like to use surveys (old named Forms Pro) and as an online service, how can we integrate to our CRM?

Do you have a hybrid environment? Potentially you can send survey to your contacts synched to D365 online then when the survey response received (at D365 online), synch the survey response to your on-prem
Our system is working on-premise environment (Not a hybrid system). As I understand, we can connect and sync to online "Customer Voice" module or service from on-premise and get response to on-premise. Is that right? Do we need an extra subscription and payment to use "Customer Voice" module on-premise?
Customer Voice responses will be stored in CDS online and you can use supported CDS data transfer mechanism to bring those data to on-prem. You will need to check with your Microsoft account to check if your on-prem subscription includes Customer Voice or you need to purchase seperately
