Customer Voice responses will be stored in CDS online and you can use supported CDS data transfer mechanism to bring those data to on-prem. You will need to check with your Microsoft account to check if your on-prem subscription includes Customer Voice or you need to purchase seperately
Our system is working on-premise environment (Not a hybrid system). As I understand, we can connect and sync to online "Customer Voice" module or service from on-premise and get response to on-premise. Is that right? Do we need an extra subscription and payment to use "Customer Voice" module on-premise?
Do you have a hybrid environment? Potentially you can send survey to your contacts synched to D365 online then when the survey response received (at D365 online), synch the survey response to your on-prem
