We have an issue where old phone call activities that have old due dates, suddenly gets recreated 10-20 times at a random time. For instance, a phone call that as created in june 2016, was suddenly recreated 23 times on the 6th of March 2018, all at the exact same time. We have checked all workflows, and we have not found anything that should cause this. There doesn't seem to be any pattern, and the users are reporting that they haven't done anything in particular that should have caused this during the time intervals in questions. Moreover, this is not a daily occurence. It seems to just happen randomly every now and then.
Has anyone experienced anything similar?

This might be a synchronization issue with Outlook, where a user is unknowingly synchronizing their tasks.
Could you verify which user is mentioned as 'Modified By' on these phone calls? And verify their synchronization settings?
