Forms Pro Survey Entity Relationship

Hi,

I have recently issued a survey using forms pro integrated with our Dynamics Marketing app.

I am attempting to create a report in Power BI showing scores by contacts in our database.

I am struggling to create a relationship between the survey activity and contactid associated with that activity.

I have followed the above but I am unsure how to link them.
