Resco Inspections Questionnaire - Change Title/Questionnaire Name on

How can I change Title/Questionnaire? - it always shows "Questionnaire" for all Questionnaires 

Can I change it to my actual Questionnaire 'name "ED Checklist" - below?

    Try changing the entity name.

    I have several Questionnaires and I want Questionnaire's name displays as a Title in Questionnaire Screen

    Changing entity name does not work for me case.

    btw, I updated the entity name - it dose not work after publishing ...

    You need to add a caption on load in the form rules of the questionnaire
