Duplicate Fields on Leads After Update 23021

Hello,

After my tenant received the 23021 update (full version 9.2.23021.00198), the Lead main form is displaying duplicate fields for the Business Phone, Mobile Phone, and Email fields. The duplicates appear on Open Leads but not on Closed Leads. I don't believe we have any custom changes for the Lead forms. A user did report the form did not have the duplicate fields on the form last week.

Is there any solution to take or should this issue be raised as a ticket? Furthermore, would this be an issue to raise to our CSP or directly to Microsoft?

    Looks like it is affecting all users on the tenant in both the Sandbox and Prod environment.

    I did replace the standard form which had the duplicated fields and removed them. The screenshot below has the newly published form on the left.

    Looks as though the fields which were duplicated only appear once after being removed from the form. I dug a bit deeper and found an extremely old solution with Lead related changes. Turns out the original person customizing the system did duplicate the fields with three hidden fields. Unsure of why the update caused the form to behave strangely but I have a way forward.

    Hi Jam,

    Is this true for all users in the h environment? Are all environments under this tenant like this?

    Maybe you can try to create one new form to replace the old.

    My environment version is also 9.2.23021.00198, but the form is normal:

    Would this be an issue to raise to our CSP or directly to Microsoft? 

    --Both are OK.

    But I only know how to open a ticket for the Microsoft support team.
