Migrating to MR 2012 - Cannot enter Company Credentials

I'm trying to migrate from FRx to MR 2012 and get to the Company Credentials (6/11) page.  

First, the 'Use same credentials' radio button is disabled.

Second, I cannot select the Windows Authentication checkbox.

Third, I'm not sure what is expected in the User Name/Password.  I've tried several different logins, including the Domain user accounts that can
login to MR.

Any ideas of what I'm missing would be appreciated.

    I believe you can skip that page by clicking Next at the bottom.

    Thanks for the reply Jake, but it will not let me past that page.  I get a message:

    "The credentials entered for one or more companies were not valid."

    In that case, I would try using the 'sa' credentials.

    Hi:

    I received in the same error when I was migrating FRx to Management Reporter.

    Prior to the migration I disabled and removed the Data Mart.

    Even though they were both disabled, the company listed was the Database name using the Data Mart Configuration.

    Verify that the Data Mart is enabled.

    Once I enabled the Data Mart and restarted they Migration, I did not receive the error message.
