This is a tough one:

I usually check in this order:

Is it deployed?

is there an active unmanaged layer on the managed system?

Are all columns on the form if a JS doesn't run?

Is the flow/JS enabled/turned on?

Can you find the flow/stack trace with an error triggering? Can you reproduce the bug?

Can you impersonate the user to reproduce the bug?

only half joking... but more is needed to help you
