    This is a tough one:
    I usually check in this order:
    1. Is it deployed?
    2. is there an active unmanaged layer on the managed system?
    3. Are all columns on the form if a JS doesn't run?
    4. Is the flow/JS enabled/turned on?
    5. Can you find the flow/stack trace with an error triggering? Can you reproduce the bug?
    6. Can you impersonate the user to reproduce the bug?
    only half joking... but more is needed to help you
