Its is a known issue which is happening for few of our clients as well. Microsoft has confirmed it as a known issue and they are working on this.
Hope this helps.
Open F12 browser developer toolbar & check for any script errors in console.
Isolate the rootcause of the issue by trying in different machine/browser/user/roles/entities/etc.
If you have any script in form/ribbon, disable the event handlers & check.
You can put breakpoints in form load, add debugger & dig further.

Go to Dynamics Admin portal and open a ticket with Microsoft Dynamics support team. You can also try clearing the browser cache or opening in Google Chrome incognito mode to see if that helps.
