Close Task is a built-in popup window with a single question - Did you Complete the task or are you Canceling it?
It's not possible to customise that form nor (because its an activity status field) is it possible to add values or change the values within it.
If you want to add more fields to filled in on task closure you will need to place them on the task form itself and find a separate means of ensuring users fill them in
