CRM Portal - sub grid Modal forms not loading

I have a sub grid on a web form step. The metadata is set up and references the Entity Form. 

When clicking the Add button, I can see the "modal-open" class gets added to the body tag.

The actual modal form does not load but you can see it in the source elements and in the Network tab's response, as below:

The form works if you load that specific URL in a new tab, I can Add entries so permissions etc seems to be all good. Its just not loading the actual Modal.

The same for Edit and Details.

I suspect this is a stylesheet/javascript issue as this is a fresh install of the Custom Portal template for D365 Online.

The following Fonts are not available also, ie. Fontawesome. The stylesheets are there but the actual font files's references seem not found.

Has anyone experience similar issues?

    I re initialized the portal instance. My issue was related to missing stylesheet, or stylesheet issues from the initial setup.

    If you view the network call in chrome dev tools, and can see the content of the modal form in chrome tools, then its similar issue.

    Hi there

    Did you manage to get to the root cause of this issue as I'm currently seeing this on a new entity form I'm working on. First time seeing this issue and everything is configured correctly (from what I'm aware).

    Like you I can load the form into it's on page and the sub-grids show as expected but within a modal (which is calling this page remotely) I'm not seeing the sub-grid data.

    I'm sure I was seeing this earlier so not sure if I've screwed something up with my JQuery but would be interesting to hear your views and outcomes.

    no changes. Completely out of the box. I created a single web page, entity form, web form steps from a custom entity. After installing, the stylesheet errora showing the missing fonts, which affected the Navbar as below. As you can see its not loading correctly out of the box. Subsequently i've removed the menu from the navbar just to get passed what its looking like below. So OOB Nav being messed up, missing fonts and Modal, hence my suspicion its something internal, part of the installation?

    If no JS error then we can rule out JS issue. So it could be related to css. Have you done any changes to themes/ bootstrap or applied any custom css?

    No JS error. Ive tried this across different browsers, ie, chrome, firefox with the same result.

    The actual form gets loaded, and can view it, interact with it via the chrome tools.

    Did you check if there are any JS error? Could it be related to browser/ popup blocking thing?
