NOT WORKING: Powerapps portal/entity form/additional settings/Action Button Placement/Above Form

Ok CRM gang, here we go:

I am creating cases from my powerapps external portal using Create Case functionality.

Everything works.  I can create a case no problem.

However, I cannot get the submit button to appear at the top of the Entity Form.  for the Entity Form, entity-form-create-case-buttons-below-form.jpgIn the Additional Settings/Action Button Placement I select "above form" (as opposed to "below form") but the submit button still shows at the bottom of the form.  I am able to position the button on the left or right side of the form.

On the internet I have only found 1 reference to this setting at: 

adx-community-eus.azurewebsites.net/.../

this is probably a bug... How do I contact Microsoft regarding this bug?
