Case Quick Create Form is not displaying

Hi, I have a Quick Create form for Case records but when I go via a subgrid and click New Case it loads the case Main form
 
The Quick Create form is included in the app and in form settings is first in the order, the table is enabled in "leverage quick create form if available". As far as I understand, there are no security roles to set for quick create forms. I have removed and readded Case from the site map navigation. 
 
Live Monitor for Form Selector shows
 
"Step 4.1: Retrieve the form id from URL parameter or programmatic input. Result: the form id --- is provided and it will take precedence over MRU (most recently used) form.",
(next steps were skipped)
 
"Step 4.5: Final validation of the form id. Result: form id --- is not accessible to current user due to security or App Module settings. The specific reason can be found in previous steps (you can search for the form id). Default system form is retrieved instead as a backup:
 
Can you help, why isn't the correct form displaying?

    Go to Power Apps > your App > Tables > Case > Forms Confirm: The Quick Create form is listed and checked and it is assigned to the app module you're using (not just enabled globally)

    Hi Ramesh, not sure I follow what you mean by listed and checked and assigned to the app module. It has been added to the app, do you mean something different?

    The Quick Create form doesn’t support security roles—it’s available to all users.
    Make sure you’ve added the form to your app under “In this app” and then save and publish the app.

    Quick Create forms require three things to work:
     
    • Quick Create forms are enabled for the table in question
    • A Quick Create form exists
    • The form has been added to your model driven app
     
    If it has been added to your app and still isn't working, make sure that the table is set to use Quick Create forms when available. (bottom right corner of my screenshot).
     
    Dataverse > Tables > Properties> Leverage Quick Create Forms
