We recently migrated from CRM 2015 to Dynamic 365 V9. After the migration, when creating a new email server profile, the ribbon for Save, Save & Close, Test & Enable... are missing. I have tried on IE, Edge, Firefox, and Chrome. All came up blank. Opening an existing email serer profile has the same result. We upgraded to V 9.0.3.7 to see if it fix it, but it did not.
Have anyone seen this issue before?

Go to Settings > Customizations > Customize the System > Entities > Email Server Profile> Forms > Main > Form Properties > Select the Display Tab. Then uncheck the Enable Form Assistant.
I believe it was the url only but we change the organization name to match.

it's just the URL or the organization should be change?
We change the url of our CRM to from CRM.COMPANY.COM to CRM123.COMPANY.COM I guess the url cannot be crm.company.com. The exact word "crm." in the url is the cause.
We were told there will be a fix coming out to fix this, but Microsoft does not know when. We can't wait for it, and we don't know if Microsoft's fix will fix the problem. We decided to just change the URL so it doesn't happen again.

I updated to update 0.6 that was recently released and now the ribbon seems to load correctly for me in IE11. Having some issues in Chrome now though - go figure.
Not sure if its the same exact issue or not but it sure sounds similar. I upgraded my CRM 2015 organization to Dynamics 365 V9.03 on prem and changed the Org name during the upgrade so my IFD URL is now crm.company.com.
Thankfully the ribbon is blank white/broken only on the Email Server Page and Mailbox Information forms as far as I can tell. Using IE to administer the new environment and changing the document mode (not the agent string), on those 2 forms is annoying but at least I can continue with testing. Hopefully this is fixed sooner than later. Do you have a Microsoft ticket number by chance that i can reference if i open one with them?

If you are anxious to fix this issue, I recommend you to submit a Premier/Unified support request.
For me changing the Document Mode from IE11 to IE 10 or IE 9 did not work. I tried all different browser version, and none work. I don't know if you have the same issue as I am. Does your Dynamic site have a URL of crm.yourcompany.com?
I was told that Microsoft schedule to release the patch sometimes in July. I don't know if we can wait that long, and if Microsoft is on schedule to release or not. For workaround, we are looking at change the site URL to something in the line of "mycompanycrm.mycompany.com". We will have to test and the new URL, custom codes, integratoin, ask user to change their Favorites. Lots of work, but I don't want to wait for Microsoft, and continue to delay my project further out.
