XML Error when adding any field to a form

Hi all,

Getting the below when trying to add a field to a form.  Worked fine in the past.

    It doesn't matter.

    I'm trying to add a simple option set to the account form but I've tested with standard fields and get the same issue.

    most probably there is a grid/field with an invalid name.
