We are getting the following error message for one user only.  Have tried numerous things including working on another PC, new windows account and new AX account.  The issue persists and we are very confused!  Please help if you can.

Try resetting Usage data for the user in System administration > Common > Users > User options button > Usage data.

After that delete the *.AUC files in the Windows User profile on the computer from where the user is running the client.

    See if that helps.

Thanks Vilmos, we have already reset the usage data in AX and deleted .AUC files from the user profile (in fact we have a script that automatically deletes them when the user logs off) but we still get this message intermittently in AX.

Your kernel version for AX 2012 R2 is 2 years behind, consider patching up all your AX components in your environment to the latest (all clients, AX AOS, SSRS/SSAS, DIXF, etc.):

That would be Microsoft Support Team's first recommendation as well.

If that does not resolve the issue, your only option is to collect a crash dump with DebugDiag or WER, and open a Support Incident with Microsoft to get it analysed to reveal the root cause of the issue.

The problem may have been resolved - instead of just resetting the usage data, i deleted every single setting for the user in system administration including all the forms etc.  We have not had a recurrence in the past week.  That said, we did go for two months previously without this problem occuring and then suddenly it occured again.  What is also very strange and goes against this resolution is that the user had a new windows account and AX account and yet the problem persisted so it will be interesting to see if this problem reoccurs later.  If so, I'll re-open the question.  Thanks for all your help.
