Unified Interface issue - the form not loading correctly.

Hi all,

i'm facing an issue now about unified, when the form loading will sometime get error message of custom code js, but i checked the code is fine. When I F5 the page, the error message show immediately, every event onchange in form is broken and not working, even the ribbon button return error.

i'm tried checking the code but didn't get anything. This issue have just happened for a week up to now.
