Voice of Customer Solution Issues - Designer Page is not opening

Hi, 

We have recently installed 'Voice of Customer' solution for Dynamics 365 Customer Service (V9).

I was trying to create a survey record and found following issues: 

1. Was able to create a survey record in 'Draft' stage and save it without any survey id value(mandatory field). 

2. When I try to open designer page , it doesn't load and it shows message as 'Working'. 

Need suggestions on how to resolve this. 

Do I need to do any configurations to populate Survey ID?
