Is there a way to display a pdf document on model driven app form? Or a way to open a note to preview not a download?
Hi Anna0326,
I believe you will have to build the PCF control in order to achieve your requirement. You can find similar control here:

This is sample PCF control that shows images and pdf from attached notes and also can show note title and note text.
1.Download and import it as solution:
2.Go to Settings > Customizations > Customize the system > Entities > Forms to open one form you need, add one Single Line of Text field and bind the control:
