I'm facing an issue with the customer service hub app. I've created a copy of new form from the existing oob form in Entitlement Entity. 

I'm facing an issue in customer service hub for the newly created form , on creation its showing an script error. However it works fine for the existing oob form. There are no custom javascripts/plugins created for the form.

Error details are as below.

TypeError: Unable to get property 'getOptions' of undefined or null reference
at allocationType_onchange

    Hi partner,

    This type of issue always occurs in JS code. Please check your entity like the following picture.

    1.Open form editor and click on "Form properties"

    Make sure that there is No js files under "Form Libraries".

    2. Please check that if there is any workflow with your Entitlement Entity.

    It will be appreciate and helpful if you could share more information about your form editor and error message.
