It usually depends upon how you provided the values during deployment of On-Prem and if you run correctly the step#14 where there is command to reset database user.
If this was working before and stopped suddenly that's a problem. If this never worked since your deployment, please review your deployment steps. especially the values you provide at time of deployment on LCS.
Which log? system, application etc. and of which VM?
