Form sent to the lead/contact and then update the fields (prefilled form would be amazing)

Hi, we want to create an easy process. If we have a lead and he want to setup a consultation with a doctor, he has to sign a document (Word Document at the moment). Basically we want him to finish his data.

We collect within the lead the following data:

  1. First Name
  2. Last Name
  3. Email Address
  4. Phone Number
  5. Birthday Date

If he wants now to setup a meeting he has to sign the document i was talking about earlier. The ideal solution would be that we send him a prewritten email with a unique link he has to click on.

Then there should be something like: Please finish your information!

He sees the information that is already in the CRM System prefilled (First Name, Last Name, Email Address, Phone Number, Birthday Date) and has more fields to fill out like:

  1. What procedure he wants - Dropdown Menu
  2. Address (City, Street Name, ...) - Single Input fields
  3. How did you find out about us? - 8 Checkboxes
  4. A few questions for us to better understand your personal situation - 9 Checkboxes
  5. And finally we have a consent where he agrees to the terms and conditions

This new data will then update the records in the CRM and we need to create a view with where we can see who's done the procedure.

My question now, how is that possible? :-)

    Hi,

    It can be achieved with Dynamics 365 Marketing.

    The link you mentioned can be done with Marketing form.

    And "the information that is already in the CRM System prefilled" can be done with the Prefill feature in marketing form. (Fore more details, please refer to Enable prefilling for marketing forms (Dynamics 365 Marketing) | Microsoft Docs)

    As for the filling of more fields, this can also be done with form fields. (Fore more details, please refer to Set up input fields for marketing forms (Dynamics 365 Marketing) | Microsoft Docs)
