Is it possible to customise Relevance Search? e.g. customise relevance score or add attribute level facets etc?

You couldn't customise relevance score logic as this is predefined by the system.
While you could customize attribute level facets as Relevance Search explorer which has Global Facets(uncustomizable) & Entity Specific Facets(customizable based on the personal)
You could refer to the 'Set up and Configuration' section in the following blog from CloudFronts which has provided step by step operation to help configure 4 facet fields for searchable entity:
