I see you have created on-demand workflow which can be only triggered manually by the user. Also it can be triggered from business process flow.
go to process ->click show dependency->check how many places you have used this workflow
Since this is not automated process so I think user has manually triggered this workflow multiple times.

1.If you mean your workflow has been triggered duplicated, you could go to System Jobs to monitor the process and find the reason.
2.If you mean the actions in this workflow duplicated, you could try the following steps.
a). Check if your conditions could be conflict.
b). Use "If...else" for your different conditions in workflow, then if one condition has been met and the action has been triggered,the other will not be met and triggered.
BTW, could you kindly share more information about your issue with English?
