Is sub grid not applicable for INSERT mode in ENTITY form?
Yes it is expected. The sub grid is not available until you create the record. This is because subgrid shows the related records to that record. If that record doesn't exists that it would not have anything to relate to.

Are you talking about adding new records to the subgrid while the form is still in Create Mode?
If that is the case, Ravi pretty much summarizes it. You cannot add a new record to a subgrid until the parent record is created.
If you are talking about adding records (creating) to a subgrid directly, that is not possible with an editable subgrid. You can create new records using a form, but you can modify them inline.
Subgrid in the form means it will show only for created record or i mean it will shouw in update form which is form type =2 and not for record which are still not created .
As runtime before create record the guid not exists to relate the data for subgrid and reason why the subgrid is not Populating.
In my past experience to overcome this issue I have added one custom HTML webresource to add a grid record and keep the data in subgrid entity database without guid and maintain another unique I'd before create and finally when you saved we are pointing the primery guid. Ifor user not created the next time we are removing the junk data for the entity.

You can add all the required subgrids on CRM form and expose it to Portal using Entity Form.
