After the upgrade to CRM Online 2015 our organization lost the ability to Qualify Leads. All managed and unmanaged solutions associated to the Lead Entity have been removed but the issue still exists. Any thoughts??
What exactly do you mean by the ability to Qualify Leads? You do not see the button Qualify Lead anymore?

The "Qualify" ribbon button still exists on the 2013 / 2015 Lead forms, however nothing happens upon click. Dynamics does not present an error message or any log files to troubleshoot. We've removed all managed and unmanaged solutions and custom JS from the form.
Qualifying a Lead from views continues to work though.
I had the same problem and I solved it. Try to restore the solution or generate MS support ticket.
May I know the browser version that you are using?
Please note CRM 2015 supports only IE 10 and above
If you want to use the Vanilla Lead Form of Dynamics CRM, I would suggest you to
1. Instantiate an Online Trial of CRM 2015,
2. Create a Solution and Add the Lead Entity without any dependent components
3. Export as an Unmanaged Solution and import it to your Organization.
This will replace your Lead Entity and you will have the Vanilla Lead Entity.

If you are in the next scenario you could apply the workaround and verify if this can solve you incident; also you could try to debug the form in and qualify button on IE and verify if you can find what is causing the blocking.
CRM 2015: It is not possible to qualify a lead inside lead form, button doesn’t work, it is possible to qualify the lead if it is executed outside lead form (Lead dashboard), if qualify button is clicked several times 404 error appears.

Company name field is necessary to enable qualify button in lead form; this is because use company name field to create account name when lead is qualified and converted to a customer account. The form needs company name field even if this is hided and there is no information on it.
1. Go to lead form
2. Customize form; add Company Name Field
3. If form doesn't need Company Name Field Hide it
If this doesn't have an effect you could try adding currency field and hide it in form too:

After a weeks effort and help from MSFT we found the jScript library called by the Qualify button was no longer valid. MSFT is unsure how/why this library changed. GOOD TIMES.
How did you reactive that?
We have exactly the same issue and the Microsoft engineer does not now how to resolve it :(
Could you please post your Microsoft Case Number, so I can send it to them ? This way, I guess they would quickly find how to solve the issue.

We just upgrade to CRM 2015 and have same issue. Could you please provide more details that how did you fix Lead Qualify issue?

Issue finally has been resolved today by Microsoft support. Our case number was: 115041491030817
A solution for the lead entity needs to be created in order to be able to customize the Ribbon. Then:
Go to settings -> solution -> open ribbon workbench -> added lead solution -> click on second qualify tab -> click on command tab -> open second script ->mscrm.form.lead.convertquick and mscrm.gridview.lead.convertquick -> and then, delete mscrm.form.lead.convertquick (right click and delete).
In any case, I recommend to contact Microsoft Support because this was a solution for us but I don't know if it would be for everybody else.
This is a strange one and one thing that seemed to cause the problem in my case at least was whether I was accessing the site over https or http. If I was using http clicking the qualify button would do nothing.

Although thread might be old, but I've caught this error couple of days ago.

Make sure that your form has 'CompanyName' and 'Subject' attributes on it.
