You can use retrievemultiple call using OData, and call filter based on phone number, here is the OData filter options: msdn.microsoft.com/.../gg309461(v=crm.7).aspx
For sample code you can use SDK.REST.js which comes with SDK and can use retrievemultiple mehtod from there, once it will record record you can get it;s record id (I hope that's what you want to get instead of entity form id)
