how do i get the dynamics for outlook to show in ribbon
Have you gone through the configuration steps? If not, search for "Configuration Wizard" and run the configuration.
If you've already gone through these steps then you can check if the addin has been disabled and reenable it through the outlook addin-screen:

I have the app installed and just the use "CRM for outlook" setting (not sure of exact phrasing as on the iPad) instead of server side syncing, it is an office 365 setup
the app still refuses to appear in the ribbon I have removed and installed a couple of times to make sure none of the older CRM was still installed before it became dynamics.

If you have server side sync setup, and permissions to the app, hit the settings wheel for your settings, then click Apps for Dynamics 365. There should be four apps lists, and one is is 'Microsoft Dynamics 365 App for Outlook'. Click through for that one, restart Outlook and you should see it (assuming you're in Outlook 2016 - 2013 doesn't have the ribbon button).
